The Division needed straightforward, accessible resources that could help patients understand the Primary Care Network while supporting physicians in addressing common misconceptions. The Ask

The Division supports family physicians and their teams by strengthening primary health care capacity and improving patient outcomes. To better communicate the purpose and offerings of the Primary Care Network, the Division engaged Monogram to design a new website and supporting educational materials.

The Division needed a user-friendly platform and clear communication tools that would both enhance patient understanding and provide physicians with practical resources to set expectations and address misconceptions.
